The Who, What, Where, When, Why, and How's of this Process

Who?

You, community residents, Plumas County Public Health staff, Plumas County public officials, Eastern Plumas Health Care, Plumas District Hospital, the Greenville Rancheria, Seneca Healthcare District, social services organizations, community benefit organizations, and many more!

What?

The Plumas County Community Health Assessment (CHA) is a report on the status of the health and well-being of Plumas County residents during 2018-2020 and 2020-2022. The Community Health Improvement Plan (CHIP) is a plan-for-action developed by a partnership of community agencies, organizations, and healthcare providers.

Where?

The CHA process involves participation from communities across Plumas County. Town Hall Meetings were held in the communities of Chester, Graeagle, Greenville, Portola, and Quincy. Focus groups and key informant interviews were conducted with groups and individuals from across the county.

When?

This project originally started in the summer of 2011. The most recent update to this work began in 2021.

Why do a Health Assessment and Improvement Plan?

  • Engage community members in health issues
  • To understand health disparities in the community
  • To differentiate the needs of various communities within the county
  • To strengthen opportunities to compete for funding opportunities
  • To collaborate with partners and meet new partners
  • To establish health priorities and promote action to achieve them

Purpose?

This will be the third assessment to understand the health issues of residents - one in 2012, one in 2016, and one planned for 2023. We're gathering information from many sources including the Town Hall meetings. From this information and your opinions, we will create a plan that outlines the key health issues, identifies priorities for improvement, and lays out the steps we will take and who will be responsible.

How?

Utilizing the model of Mobilizing for Action through Planning and Partnerships developed by NACCHO, we will follow a structured process to assess and prioritize needs, develop and implement measures, evaluate the plans and continuously improve the process.

Social Determinants of Health:

Why is it important to discuss what affects health and how we can improve these factors?

Health begins where we live, work, and play, in our schools, homes, and jobs. According to the Robert Wood Johnson Foundation, "All Americans should have the opportunity to make the choices that allow them to live a long, healthy life, regardless of their income, education or ethnic background."
