Passport Check List
Every applicant must appear in person and have the following:
- Passport Form DS11-Filled Out-Signed in the presence of the Passport Agency
- One Passport Picture (Locally: Kimberly Caroll - (530) 249-2307 cell, or madsam97@sbcglobal.net)
- Certified Copy of Birth Certificate or Previous Passport
- Photo Identification - (Ex: Drivers License) Both parents must present ID for those under 16, one parent over 16.
- Check or Money Order to: U.S. Department of State. $130.00 per passport.
- Minor under age 16 $100.00 per passport - Must be check or money order
- Check, Money Order, or Cash: Plumas County Treasurer. $35.00 per passport
To Expedite: USPS Flat Rate Envelope with Attached $31.40 (amount subject to change at any time) Postage (Pickup Prior) US Department of States Fee $209.53 per adult passport, $179.53 per minor passport.
Plumas County Treasurer Fee $35.00 per passport
US Passport Card: Can be used for Mexico, Canada, the Caribbean, and Bermuda (Not for Air Travel)
Adults - $30.00 Minors - $15.00 Plumas County Treasurer - $35.00
